© Minnesota Voice and Speech Clinic 2010 Insurance and Financial Information We have provider agreements with the following health plans: Blue Cross Blue Plus Medica Preferred One Patient Choice UCare You should contact your insurer about your specific plan coverage for speech therapy services or any restrictions.  Since most health plans require a physician referral for voice, speech, or swallowing therapy services, a physician signature and reason for referral is required before beginning services at our office. If your plan requires a prior authorization, you will need to obtain this before your first visit. Some group plans may have a provider arrangement with another agency or provider. If your insurance plan has a specific deductible and/or co-payment for speech therapy services you will be responsible for this cost. It is important to know that some insurance plans do not consider some speech diagnoses/problems as "medically necessary" and therefore will not cover services.  We will provide reports and information to you insurer to help obtain benefits. However, the patient (if minor, the parent or guardian) is responsible for the charges incurred. To help us reduce the costs of providing therapy we ask that all of our patients pay their deductibles and co-payments at the time of the visit. If you need to make payment arrangements, please do so prior to your scheduled appointment. We believe that we offer therapy services that can provide desirable outcomes for our patients. We are convinced that speech therapy is a valuable and worthwhile, even if the costs are not paid by an insurer or other third party payor. Our insurance billing is handled by our contracted service, which provides secure electronic submission of our claims.
